Life Empowerment Services of Marietta, GA has partnered with The Sparrow Center of Albany, GA to provide PUP service assessments/therapy for Region 10 counties and we would love to work with you going forward. Case Managers may use one of the following options to submit service authorizations for substance abuse assessments, parental fitness assessments, trauma assessments, domestic violence assessments, or counseling services:

Option 1: Email service authorization to ALL of the following emails: dfcsreferrals@lifeeservices.com and admin@sparrowcenter.site and ahall@sparrowcenter.site

OR

Option 2: Submit service request on ARGO and select Life Emowerment services as the accepting agency.
